To provide information to the Public Works Department on a road maintenance problem within the unincorporated areas of Brevard County (i.e., potholes, road failure, drainage problems, etc.), please complete the following form. This information will be
automatically directed to the Public Works Road & Bridge Public Access Service Request system for processing and review.
Once the form is submitted an auto-confirmation email will be sent to the email address provided. The email will contain a Service Request number and a Web Link to track your Service Request. Please use the link contained in that email to check the status
of your Service Request at any time. Always provide the Service Request number when directly contacting Public Works operations staff. Please allow a minimum of three (3) business days to respond to your request. Only one email will be sent from this
Service Request system.
This form SHOULD NOT be used to report emergencies or other serious problems. For emergencies During Business Hours, please contact the Public Works Department directly at (321) 617-7202. After hours emergencies should
be reported to (321) 637-6670.
Please DO NOT use this form for problems/complaints for any roads with a State Road Designation (US1, I-95, etc.) or on a city-maintained street. State Roads fall under the jurisdiction of the Florida Department of Transportation (FDOT).
